Nurse Practitioner/Physician Assistant – Health Center in Albion, New York (Part-Time/Full-Time Opportunities)

Position Summary:

We are seeking a Nurse Practitioner to provide full-scope primary care services in collaboration with a Primary Care Physician. This includes diagnosis, treatment, care coordination, preventative care, and health maintenance for patients. As a key member of this health centers team, the Nurse Practitioner will collaborate with fellow team members to foster an environment of trust and mutual respect. The role requires compliance with the health center's policies, state and federal regulations, and maintaining the highest standard of patient care.

Key Responsibilities:

  • Collaborate with MD/DO to guide medical decision-making within the Care Team
  • Perform History of Present Illness (HPI) and physical examinations
  • Diagnose conditions and develop treatment plans for patients
  • Prescribe medication following New York State guidelines and professional practice standards
  • Perform minor surgical procedures as necessary
  • Generate referrals for patients requiring services outside primary care
  • Review patient-related data (reports, telephone encounters, labs, imaging, etc.) and direct follow-up care promptly
  • Document patient interactions accurately and in a timely manner, as per employment contract
  • Adhere to scheduling and productivity standards
  • Participate in outreach and off-site healthcare programs within the health service area, as assigned by the Chief Medical Officer
  • Follow the Infection Control plan, ensuring all staff comply with universal precautions
  • Participate in peer reviews and quality assurance projects
  • Regularly attend departmental, provider, staff, and other required meetings
  • Assist in updating protocols and practices
  • Represent the health center in external medical public relations events
  • Maintain an unrestricted New York State medical license and all required certifications (including DEA and narcotics licensure)
  • Keep Board Certification in the area of specialty current
  • Complete Continuing Medical Education (CME) to stay updated on evidence-based primary care practices
  • Perform any other duties as requested by management

Skills and Qualifications:

  • Computer proficiency and familiarity with electronic health records
  • Strong proficiency in MS Word and Outlook
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively within a healthcare team

Education and Experience:

  1. Current New York State medical license
  2. Current DEA certificate
  3. Board-eligible or board-certified; if board-eligible at the time of hire, must achieve board certification within 18 months of hire
  4. Current Cardiopulmonary Resuscitation (CPR) certificate
